Create detailed plans for transitioning to independent living through multi-agency support and collaboration.
Safety and Well-being in a Supportive Environment
Ensure the safety and well-being of young people with complex needs in a nurturing and supportive setting, fostering positive family attachments.
Skill Enhancement and Aspiration Support
Provide sessions to enhance independent living skills and support educational and employment aspirations.
